Hangar Change Request
ACORD 342, Hangar Change Request, is used to submit requested
changes to a hangar policy. This form may stand on its own or be attached to the ACORD
335, Aviation Policy Change Request Applicant Information Section.
IDENTIFICATION SECTION Agency Customer ID
Enter identifier: The customer's identification number assigned by the producer (e.g.
agency or brokerage).

Enter date: The month/day/year on which the form is completed. (MM/DD/YYYY)
